Revised approach suggestion
Hi all this topic has gone quiet in the last few weeks as we are all incredibly busy. I've just had time to get back to thinking about this and have put together very roughly a suggested new format for the checklist. I've taken on board Matthew's suggestion that everything should be in one document, and also tried to retain the format of a section for the decision maker, which comprises the first two pages, section for the evaluator, which is the bulk of the document, and a section for our workflow processes which forms the backend of the document. This is a very rough draft - which focuses more on the structure than the content - due to time constraints so suggestions and contributions are very welcome.
