Member Join Form

Hi, I made some revisions to this formal member join form. I'm not sure I made it better. My aim was to simplify the word choices.

However, it's still too long. I would like us to choose the TOP 3-5 commitment statements from this list, and see if we can't eliminate the other ones without losing the essence of what membership should mean in Shared Ground.
